چکیده: (357 مشاهده)
زمینه: درد مزمن بهعنوان یک پدیده زیستی–روانی–اجتماعی، نهتنها با عوامل جسمانی بلکه با تجربیات رشدی اولیه و متغیرهای شناختی–هیجانی مرتبط است. با وجود تأیید نقش آسیبهای دوران کودکی در بروز مشکلات روانشناختی و نیز اهمیت متغیرهای شناختی در تجربه درد مزمن، پژوهشهای پیشین عمدتاً این عوامل را بهصورت مجزا بررسی کردهاند و سازوکارهای میانجی تبیینکننده این روابط کمتر مورد توجه قرار گرفته است.
هدف: پژوهش حاضر با هدف بررسی رابطه بین آسیبهای دوران کودکی و پریشانی روانشناختی در افراد مبتلا به درد مزمن و نقش میانجی خودکارآمدی درد و فاجعه سازی درد انجام شد.
روش: این پژوهش از نوع توصیفی و همبستگی بود. جامعه آماری شامل تمامی افراد مبتلا به درد مزمن (گوارشی، قلبی، اسکلتی) شهر نیشابور در سال 1403 بود که با استفاده از روش نمونه گیری دردسترس 255 نفر به عنوان نمونه انتخاب شدند. ابزارهای پژوهش شامل پرسشنامههای آسیب دوران کودکی (برنستاین، 2003)، پریشانی روانشناختی (کسلر، 2002)، خودکارآمدی درد (نیکولاس، 2007) و فاجعهسازی (سالیوان و همکاران، 1995) بود. دادهها با استفاده از ضریب همبستگی پیرسون و تحلیل مسیر در نرمافزار SPSS-27 و AMOS-24 مورد تحلیل قرار گرفتند.
یافتهها: نتایج نشان داد که بین آسیب دوران کودکی، خودکارآمدی درد و فاجعه سازی درد و پریشانی روانشناختی رابطه معنادار وجود دارد (05/0 >p). اگرچه اثر مستقیم آسیبهای دوران کودکی بر پریشانی روانشناختی معنادار نیست، اما این اثر بهصورت غیرمستقیم از طریق خودکارآمدی درد و فاجعه سازی درد معنادار است (05/0 >p).
نتیجهگیری: نتایج نشان میدهد که تأثیر آسیبهای دوران کودکی بر پریشانی روانشناختی بیماران مبتلا به درد مزمن، عمدتاً از مسیر متغیرهای شناختی قابل مداخله تبیین میشود. ازاینرو، طراحی و اجرای مداخلات بالینی متمرکز بر تقویت خودکارآمدی درد و کاهش فاجعهسازی درد میتواند راهبردی مؤثر برای کاهش پریشانی روانشناختی این بیماران، بهویژه در افرادی با سابقه آسیبهای اولیه، باشد.
