اثربخشی درمان تحریک الکتریکی مستقیم فراجمجمهای مغز بر عاطفه خودآگاه شرم و گناه و حساسیت انزجاری نوجوانان مبتلا به اختلال وسواسی-جبری: یک مطالعه تک آزمودنی
The effectiveness of transcranial direct current stimulation on the self-conscious affections of shame and guilt and the disgust sensitivity of adolescents with obsessive- compulsive disorder: A single case study

<div style="text-align: justify;"><span style="font-family:Tahoma;"><span style="font-size:11pt"><span style="line-height:normal"><span style="direction:rtl"><span style="unicode-bidi:embed"><b><span lang="FA" style="font-size:10.0pt"><span style="color:#a42898">زمینه: </span></span></b><span lang="FA" style="font-size:10.0pt">تحریک الکتریکی مستقیم فراجمجمهای، یک تکنیک تحریک تعدیل عصبی غیرتهاجمی با تحمل خوب و بدون عوارض جانبی در درمان و بهبود نشانههای وسواسی است که قابلیت تحریکپذیری قشری را به طور مؤثری تعدیل میکند. با این حال، هنوز شواهد قوی و کافی برای استفاده از این روش درمانی در نوجوانان مبتلا به اختلال وسواسی-جبری وجود ندارد.</span></span></span></span></span><br>
<span style="font-size:11pt"><span style="line-height:normal"><span style="direction:rtl"><span style="unicode-bidi:embed"><b><span lang="FA" style="font-size:10.0pt"><span style="color:#a42898">هدف:</span></span></b><span lang="FA" style="font-size:10.0pt"> پژوهش حاضر با هدف بررسی اثربخشی درمان تحریک الکتریکی مستقیم فراجمجمهای بر عاطفه خودآگاه شرم و گناه و حساسیت انزجاری نوجوانان مبتلا به اختلال وسواسی-جبری انجام شد.</span></span></span></span></span><br>
<span style="font-size:11pt"><span style="line-height:normal"><span style="direction:rtl"><span style="unicode-bidi:embed"><b><span lang="FA" style="font-size:10.0pt"><span style="color:#a42898">روش:</span></span></b><b> </b><span lang="FA" style="font-size:10.0pt">در این پژوهش از روش تجربی از نوع تک موردی با طرح سنجش پیوسته و آزمودنیهای متفاوت استفاده شد. جامعه آماری شامل نوجوانان مبتلا به اختلال وسواسی-جبری مراجعه کننده به مرکز مشاوره یاس شهر تبریز بودند که از بین آن­ ها، تعداد 4 نفر به روش نمونهگیری هدفمند انتخاب شدند و بر اساس پروتکل گرین و همکاران (2020) تحت مداخلۀ ده جلسهای درمان تحریک الکتریکی مستقیم فراجمجمهای به صورت هفتهای دو بار و هر بار به مدت 20 دقیقه قرار گرفتند. آزمودنیها در مراحل خط پایه، جلسات دوم، چهارم، ششم، هشتم، دهم و یک ماه پیگیری با مقیاس حساسیت انزجاری (هیدت، مککوالی و روزین، 1994)، پرسشنامه احساس گناه (کوگلر و جونز،1992) و پرسشنامه احساس شرم (کوهن، وولف، پانتر و اینسکو،2011) مورد ارزیابی قرار گرفتند. جهت تحلیل دادهها روش ترسیم دیداری، شاخص تغییر پایا، درصد بهبودی و درصد دادههای ناهمپوش استفاده شد.</span></span></span></span></span><br>
<span style="font-size:11pt"><span style="line-height:normal"><span style="direction:rtl"><span style="unicode-bidi:embed"><b><span lang="FA" style="font-size:10.0pt"><span style="color:#a42898">یافته­ ها:</span></span></b><b> </b><span lang="AR-SA" style="font-size:10.0pt">نتایج نشان داد که درمان تحریک الکتریکی مستقیم فراجمجمهای </span><span lang="FA" style="font-size:10.0pt">در کاهش</span><span lang="AR-SA" style="font-size:10.0pt"> احساس شرم و گناه و حساسیت انزجاری در نوجوانان با اختلال وسواس</span><span lang="FA" style="font-size:10.0pt">ی-جبری </span><span lang="AR-SA" style="font-size:10.0pt">تأثیر معناداری دارد (</span><span dir="LTR" style="font-size:8.0pt">RCI≥1/96</span><span lang="AR-SA" style="font-size:10.0pt">) و اثرات درمانی در مرحلۀ پیگیری نیز حفظ شده است.</span></span></span></span></span><br>
<b><span dir="RTL" lang="FA" style="font-size:10.0pt"><span style="line-height:107%"><span style="color:#a42898">نتیجهگیری:</span></span></span> </b><span dir="RTL" lang="FA" style="font-size:10.0pt"><span style="line-height:107%">نوجوانان مبتلا به اختلال وسواسی-جبری که تحت درمان تحریک الکتریکی مستقیم فراجمجمهای قرار گرفتند، احساس شرم و گناه و حساسیت انزجاری کمتری را تجربه کردند، استفاده از این روش در درمان نوجوانان مبتلا به اختلال وسواسی-جبری پیشنهاد میشود.</span></span></span></div>
<div style="text-align: justify;"><span style="font-size:11pt"><span style="line-height:normal"><span style="unicode-bidi:embed"><span style="font-family:Calibri,sans-serif"><b><span style="font-size:10.0pt"><span style="font-family:"Times New Roman",serif"><span style="color:#a42898">Background:</span></span></span></b> <span style="font-size:10.0pt"><span style="font-family:"Times New Roman",serif">Transcranial direct electrical stimulation is a non-invasive neuromodulation stimulation technique with good tolerance and no side effects in the treatment and improvement of obsessive-compulsive symptoms, which effectively modulates cortical excitability. However, there is still no strong and sufficient evidence for the use of this treatment method in adolescents with obsessibe- compulsive disorder.</span></span></span></span></span></span><br>
<span style="font-size:11pt"><span style="line-height:normal"><span style="unicode-bidi:embed"><span style="font-family:Calibri,sans-serif"><b><span style="font-size:10.0pt"><span style="font-family:"Times New Roman",serif"><span style="color:#a42898">Aims:</span></span></span></b><span style="font-size:10.0pt"><span style="font-family:"Times New Roman",serif"> The present study was aimed at the effectiveness of transcranial direct current stimulation on the Self-conscious Affections of Shame and Guilt and the disgust sensitivity<b> </b>of Adolescents with obsessibe- compulsive disorder.</span></span></span></span></span></span><br>
<span style="font-size:11pt"><span style="line-height:normal"><span style="unicode-bidi:embed"><span style="font-family:Calibri,sans-serif"><b><span style="font-size:10.0pt"><span style="font-family:"Times New Roman",serif"><span style="color:#a42898">Methods:</span></span></span></b><span style="font-size:10.0pt"><span style="font-family:"Times New Roman",serif"> In this research, the experimental method was used as a single case method with a continuous measurement design and different subjects. The statistical population of the present study included all teenagers suffering from obsessive-compulsive disorder who were referred to Yas Counseling Center in Tabriz in 1400, and 4 of them were selected by purposive sampling and based on the protocol of Green et al (2020) they underwent ten sessions of transcranial direct current stimulation twice in a week for 20 minutes each time. Subjects in the baseline stages, the second, fourth, sixth, eighth, tenth, and one-month follow-up sessions with disgust sensitivity scale (Heidet, McCauley, and Rosin, 1994), guilt questionnaire (Kugler and Fons, 1992) and shame questionnaire (Cohen, Wolff, Penter and Insko, 2011) were evaluated. For data analysis, a visual drawing method, reliable change index, percentage of recovery, and percentage of non-overlapping data were used.</span></span></span></span></span></span><br>
<span style="font-size:11pt"><span style="line-height:normal"><span style="unicode-bidi:embed"><span style="font-family:Calibri,sans-serif"><b><span style="font-size:10.0pt"><span style="font-family:"Times New Roman",serif"><span style="color:#a42898">Results:</span></span></span></b><b> </b><span style="font-size:10.0pt"><span style="font-family:"Times New Roman",serif">The results showed that transcranial direct electrical stimulation has a significant effect on reducing feelings of shame and guilt and disgust sensitivity in adolescents with obsessive-compulsive disorder (RCI≥1/96), and the therapeutic effects are maintained in the follow-up phase</span></span><span dir="RTL" lang="FA" style="font-size:10.0pt"><span style="font-family:"Times New Roman",serif">.</span></span><span style="font-size:10.0pt"><span style="font-family:"Times New Roman",serif"></span></span></span></span></span></span><br>
<b><span style="font-size:10.0pt"><span style="line-height:107%"><span style="font-family:"Times New Roman",serif"><span style="color:#a42898">Conclusion:</span></span></span></span></b><span style="font-size:10.0pt"><span style="line-height:107%"><span style="font-family:"Times New Roman",serif"> Adolescents with obsessive-compulsive disorder who were treated with transcranial direct electrical stimulation experienced less shame and guilt and less disgust sensitivity. It is recommended to use this method in the treatment of adolescents with obsessive-compulsive disorder.</span></span></span><div></div></div>
