Background: Given the increasing prevalence of marital conflicts and the decisive role of sexual components in couples’ relationship quality, identifying and implementing effective interventions aimed at regulating sexual cognitions and emotions has become increasingly important. Accordingly, a therapeutic focus on the regulation and modification of sexual fantasies may offer a novel and practical approach to enhancing marital functioning among conflicted couples.
Aims: The present study aimed to evaluate the effectiveness of a Sexual Fantasy Regulation and Modification intervention package on marital expectations, sexual intimacy, sexual self-concept, and sexual satisfaction among conflicted couples.
Methods: This applied study employed a quasi-experimental design with pretest, posttest, and a three-month follow-up, including a control group. The statistical population consisted of conflicted couples who referred to counseling centers affiliated with the Iranian Organization of Psychology and the State Welfare Organization in Kermanshah from winter 2022 to spring 2023. From this population, 16 couples (32 individuals) were selected through convenience sampling and randomly assigned to experimental and control groups. Research instruments included the Marital Expectations Questionnaire (Park, 2012), the Sexual Intimacy Questionnaire (Batlani et al., 2010), the Multidimensional Sexual Self-Concept Questionnaire (Snell, 1995), and the Hudson Sexual Satisfaction Questionnaire (Hudson et al., 1981). The experimental group received the Sexual Fantasy Regulation and Modification intervention (Alibakiyan et al, 2026) in 11 weekly 90‑minute sessions delivered within an integrative couple therapy framework, while the control group received no intervention. After examining statistical assumptions, data were analyzed using mixed-design repeated-measures analysis of variance (ANOVA) with SPSS version 26.
Results: The results of the mixed-design repeated-measures ANOVA indicated that the Sexual Fantasy Regulation and Modification intervention had a statistically significant and very strong overall effect on marital expectations, sexual intimacy, sexual self-concept, and sexual satisfaction (p < 0.001). Interaction analyses revealed significant improvements in these variables in the experimental group compared with the control group at both posttest and follow-up stages. Bonferroni post hoc tests further confirmed the relative stability of the intervention effects over time.
Conclusion: The findings suggest that the Sexual Fantasy Regulation and Modification intervention is an effective and culturally adapted approach for enhancing marital relationship quality. Through concurrent mechanisms of emotion regulation, cognitive restructuring, modification of sexual scripts, and the strengthening of sexual communication, this intervention can lead to improvements in marital expectations, sexual intimacy, sexual self-concept, and sexual satisfaction among conflicted couples.
